 Location:
We're on Burnside, which divides north from south Portland, on the west side of the Willamette River. We share parking with Zupans just west of 23rd in the Uptown Shopping Mall. Our small stand-alone building is west of the Chase Bank building, on the west side of Zupans.
**If you anticipate spending more than one hour at our clinic, please ask our front desk receptionist for a patient parking pass. **
Directions:
From east or north: Get onto I-405 heading south after crossing the Williamette, take the Burnside exit. Go through the 1st stoplight and turn right onto Burnside at the 2nd stoplight. Continue west just past 23rd where you can turn into the Zupan's parking lot.
From west or south (I-5 or Highway 26): Take the I-405 exit and then the Everett St. Exit. Turn left onto Glisan and go west. Turn left onto 23rd and go across Burnside. Go into the parking lot in front of the Zupan's to park.
 Preparing for your appointment:
- Allow 1 -1.5 hours for a complete comprehensive examination
- Wear your contacts in for your appointment
- Bring in spectacle and contact lens prescriptions (or contact lens box tops) along with your current glasses
- Bring a list of current medications, medical insurance card and the name of your family physician
- Be prepared to have your eyes dilated
Insurances/Payment Policy:
- Our office participates in almost every major insurance plan in the Portland metropolitan area. We will gladly bill your insurance for you if we can verify your coverage and benefits.
- Please be prepared to pay co-payments, deductibles, charges for non-covered services when received, and for non-covered materials when ordered. Also, if we are unable to verify your benefits, be prepared to pay in full and be reimbursed by your insurance if applicable.
- Due to the custom-made nature of our product, refunds are generally not available.
